HOW YOU'LL MAKE A DIFFERENCE: As a Registered Nurse Case Manager (RNCM) you will make a difference in the lives of our patients. Our nurses give our patients the greatest gift the ability to spend enhanced quality time with their loved ones in their preferred environment. You will provide one?on?one patient care, coordinate care, and deliver home?based nursing care as directed by an attending physician. You will be part of an interdisciplinary team that focuses on compassionate quality care and producing positive outcomes for your patient population.

HOW YOU'LL WORK: Youll assume ultimate accountability and leadership for the assessment of the patient condition and plan of care. You will case manage and provide clinical care under the direction of the physician. Youll educate patients and their family members and ensure the safety of the patient. You will work autonomously and manage your time efficiently. All practices and duties are within the scope of practice outlined by the state.
Major Areas Of Responsibility:
Plan of Care: Observe and monitor patient conditions, perform OASIS assessments, develop individualized care plans, and adjust as needed.
Minimize Patient Risk: Administer medication as prescribed, prioritize visits for high?risk patients, and reduce re?hospitalizations.
Supervision: Oversee and supervise total care of patients provided by nurse aides and LPNs.
Continuity of Care: Manage multidisciplinary care, promote continuity of care with appropriate admissions, transfers, and discharges.
Family Educator/Advocate: Educate patient and family on disease process using teach?back methods.
Policies: Complete clinical documentation following agency protocol and Medicare/Federal guidelines. Collaborate with interdisciplinary team and physicians to ensure optimal care.
Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations. Communicate changes in schedule/availability to schedulers or supervisors.
Operations: Attend in?service training and mandatory agency meetings.

Patience is a virtue when working with patients, families, physicians, and coworkers.
Attention to detail is critical, as is being observant and following directions.
Requirements:
Registered Nurse with current license in the state of employment.
Minimum one?year experience as an RN in an acute care setting. Home care experience preferred.
Valid drivers license and auto insurance in your name as a driver.
Capable of all physical demands.
