Registered Nurse (RN) Case Manager
Work under general direction with physicians and other multidisciplinary team members to develop plans for care for designated patients. Ensure patients are progressing toward desired outcomes by monitoring care through assessments, patient and family interactions and review of patient records. Be able to identify and resolve barriers that hinder effective and timely patient care and discharge. Advocate for the patients and be actively involved with our amazing Social Work team and others in the discharge planning process.

Registered Nurse (RN) Case Manager Job Duties:

Assures (and performs, as necessary) completion of a physical, mental, and psychosocial assessment within approximately 24 hours of admission to facilitate the formulation of an effective discharge plan.

Throughout the patient’s admission, effectively communicates with the multidisciplinary team to facilitate an effective discharge plan.

Documents assessments and interventions in the patient’s medical record.

Assists patients/families in coping skills and provides emotional support.

Discusses financial alternatives and implications of options with patients and families, utilizing the appropriate resources from Utilization Review and other areas.

Participates in and leads, as necessary, interdisciplinary meetings.

Serves as resource person for the multidisciplinary team.

Works closely and effectively with social service teammates to assure an effective discharge plan with appropriate resources.

Is a liaison between Froedtert South and external referral sources.

Attends and presents in-service and educational programs in order to maintain and enhance professional expertise.

Receives and records verbal orders from licensed independent practitioners for orders that are specific to the services provided by their department.

Registered Nurse Case Manger Minimum Requirements:

RN Wisconsin license

3-5 years of hospital clinical experience as a registered nurse

Proven ability to function effectively with others to develop and implement a plan to address the needs of the patient during hospitalization and after discharge

Licensure to practice as a Registered Nurse in the State of Wisconsin.

Nursing experience in the healthcare arena with previous clinical experience.

Must be able to read, write, speak, hear and comprehend the English language.
