Position Overview
We are currently seeking a Full Time RN Case Manager for the Bridgeport through East Haven Areas. Lighthouse Home Health Care is a growing company and in need of Registered Nurses that want to be part of a collaborative team environment. We are looking for nurses that have experience and knowledge of home care which is one of the fastest growing industries in the country.
Job Schedule: Full Time, 32 hours/week.
Responsibilities
Knowledge of the OASIS process.
Knowledge of Community Resources.
Ability to make meaningful observations, evaluations and reports.
Provide skilled nursing care based on scientific principles.
Maintain therapeutic relationships with patients, caregivers, and multidisciplinary team.
Perform assessments, plan of care, documentation, and care coordination.
Requirements
Minimum 1 year Home Care Experience.
Comprehensive knowledge and skill in current nursing practice.
Knowledge of the concept of team and primary care nursing and the implementation of both models of care delivery.
Must possess a Nursing degree from an accredited College or University and be a graduate of a CT approved RN program.
Valid driver’s license/car insurance.
Active RN License in the State of CT.
Proof of Covid‑19 Vaccination.
Excellent communication skills in English.
Physical ability to bend, push, pull, lift, and perform required duties.
