Registered Nurse RN Case Manager Professional Case Management is seeking a skilled Registered Nurse to serve as an RN Case Manager for home health clients who are former nuclear weapons workers with chronic and terminal illnesses.

$42 per hour (including 100% of hourly wage paid for drive time).

Key Responsibilities Conduct weekly in?home nursing visits (12 hours per visit) for clients.
Perform physical assessments, record vitals, and ensure home safety with medical equipment.
Fill medication planners and oversee skilled and non?skilled caregivers.
Provide direct care and case management of assigned clients.
Collaborate with regional director and other personnel to improve services.
Participate in client care conferences and maintain clinical documentation.
Adhere to nursing delegation guidelines and agency policies.
Assist with marketing activities and agency performance improvement initiatives.
Qualifications Graduate of a state?approved school of professional registered nursing; BSN preferred.
Current, unrestricted RN license in the state(s) of practice.
Minimum of 2 years of nursing experience, including 1 year in home care or a related field.
One year of supervisory and/or case management experience preferred.
Current CPR certification.
Additional Information Standard rate: $42.00 per hour plus shift differentials where applicable.
