Location: Tacoma, WA

Schedule: Monday–Friday, 0600–1800

Key Responsibilities

Conduct patient assessments and develop care plans.

Coordinate interdisciplinary care and discharge planning.

Manage high-complexity patient caseloads.

Support outcome evaluation and family education.

Qualifications

Graduate of an ACEN/CCNE-accredited nursing program.

Active, unrestricted RN license.

Certification in CCM, CRRN, COHN, or equivalent preferred.

Minimum 1 year of case management experience.

BLS certification.

Position Contingent upon contract award. Anticipated fall start
