We provide in‑home care to former Nuclear Weapons Workers suffering from chronic and terminal illnesses due to occupational exposures.
As an RN‑Case Manager you will conduct in‑home nursing visits, perform physical assessments, ensure home safety, develop medication plans, and oversee care teams. Visits are typically 1‑2 hours each, weekly.
Responsibilities
Provide direct care and case management to assigned clients.
Plan, implement, and evaluate care; coordinate client care conferences.
Serve as an on‑site clinical resource ensuring client home care needs are met.
Assist staffing and scheduling to meet client needs and staffing standards.
Adhere to agency policies, nursing delegation guidelines, and scope of practice.
Review clinical documentation for accuracy and completeness.
Participate in agency performance improvement initiatives and data collection.
Conduct supervisory visits and review nurse shift reports for performance improvement.
Assist with marketing activities as requested.
Perform additional duties as assigned.
Qualifications
Graduate of a state‑approved school of professional registered nursing; BSN preferred.
Current, unrestricted RN license in the state(s) of practice.
Minimum of two (2) years nursing experience, including at least one (1) year in home care or a closely related field.
Preferred one (1) year of supervisory or case‑management experience.
Current CPR certification.
