About The Role We’re seeking a compassionate Registered Nurse (RN) to join our hospice team—available full‑time, part‑time, or PRN. As an RN Case Manager, you’ll provide direct patient care, coordinate individualized care plans, and serve as a trusted presence for patients and families. You’ll collaborate with an interdisciplinary team to ensure every patient is cared for—body, mind, and spirit.
How You’ll Make a Difference Assess patient and family needs and develop personalized care plans.
Deliver skilled, compassionate nursing care and symptom management.
Administer medications and coordinate care across the healthcare team.
Educate and support families to promote comfort and peace of mind.
Lead and guide hospice aides to ensure consistent, compassionate care.
Maintain accurate documentation and participate in ongoing education.
What You’ll Bring Current RN license in Virginia (multi-state or single-state).
CPR certification and at least two years of nursing experience (hospice or home health preferred).
Strong clinical judgment, communication, and organizational skills.
Reliable transportation, valid driver’s license, and insurance.
