Assignment Details:

  • Start Date: November 24, 2025
  • End Date: March 21, 2026
  • Contract Type: Full-time, Temp-to-Perm (opportunity to extend or convert based on business needs)
  • Location: Onsite – Grand Blanc, MI (must include location on resume)
  • Pay Rate: $55 per hour (weekly pay)
  • Schedule: 4x10-hour shifts; 7:30 AM – 6:00 PM with weekend rotation required
  • Department: Case Management

Position Summary

The RN Case Manager supports physicians and the interdisciplinary care team in facilitating high-quality, cost-effective patient care. This role integrates utilization management, discharge planning, and patient education to promote efficient care coordination, ensure appropriate resource use, and enhance patient satisfaction and outcomes.

Responsibilities

  • Coordinate the collaborative management of patient care across the continuum, addressing and removing barriers to timely care delivery and reimbursement.
  • Facilitate precertification and authorization processes with payors.
  • Participate in discharge planning and ensure smooth transitions of care for patients.
  • Utilize process improvement methods to evaluate and enhance patient outcomes.
  • Support and coach staff in clinical documentation to ensure accurate representation of patient acuity and service intensity.
  • Serve as a resource for physicians, coders, and care teams regarding clinical documentation and case management practices.
  • Communicate effectively with physicians, nurses, patients, families, and payors to optimize care plans.
  • Maintain a current understanding of managed care, reimbursement principles, and hospital utilization policies.
  • Perform all case management functions both on the patient floor and within the care management system.

Minimum Qualifications

  • Education: Bachelor of Science in Nursing (BSN) – required.
  • Licensure: Current and valid Registered Nurse (RN) license in the State of Michigan – required.
  • Certifications: Basic Life Support (BLS) certification through the American Heart Association – required.
  • Experience:
  • Minimum 5 years of acute inpatient case management experience required.
  • Experience with acute discharge planning required.
  • Prior experience using CarePort system required.
  • Experience with AllScripts, GEMS, or Sunrise electronic charting systems preferred.
  • Previous utilization management and managed care reimbursement experience preferred.
  • Skills:
  • Strong organizational, communication, and negotiation skills.
  • Excellent time management and ability to prioritize multiple cases.
  • Proficient in data management and PC applications.
  • Capable of working independently and exercising sound clinical judgment.
  • Comfortable engaging directly with patients on the floor and adapting quickly to new environments.
