$40/hr (including 100% of Hourly Wage Paid for Drive Time)

We provide in-home care to former Nuclear Weapons Workers suffering from chronic and terminal illnesses due to their previous work environment.

Our RN-Case Managers conduct weekly in-home nursing visits, lasting 1-2 hours, including physical assessments, home safety evaluations, medication planning, and oversight of caregivers.

Enjoy one-on-one patient relationships and the satisfaction of improving clients’ health and quality of life. Flexible scheduling allows time for personal activities.

Qualifications

Graduate of a state-approved school of professional registered nursing

BSN preferred

Current, unrestricted RN license in the state(s) of practice

Minimum of two years nursing experience, including one year in home care or a related field

One year of supervisory or case management experience preferred

Current CPR certification

Essential Functions/Areas Of Accountability

Provide direct care and case management of clients

Collaborate with the regional director and staff to improve services

Plan, implement, and evaluate care, and coordinate client care conferences

Serve as a clinical resource and support home care needs

Assist with staffing and scheduling within scope of practice

Adhere to nursing delegation guidelines and agency policies

Participate in hiring, training, and evaluating clinical staff

Review clinical documentation for accuracy

Contribute to performance improvement initiatives

Collect and document data on infections, complaints, and activities

Perform supervisory visits and review nurse shift reports

Ensure home chart completeness and equipment tracking

Participate in marketing activities as needed

The senior case manager shall be available during operating hours for relevant activities

Perform additional duties as necessary
