Registered Nurse (RN) Case Manager – PCM/CCM Care (Bay Area, Hybrid)

Salary: $140,000 per year

Location: San Francisco Bay Area (Hybrid / Remote flexibility)

About the Role

We’re hiring an experienced Registered Nurse (RN) Case Manager to coordinate Principal Care Management (PCM) and Chronic Care Management (CCM) services for patients with chronic and neurological conditions. You’ll lead Licensed Practical Nurses (LPNs), manage care plans, and ensure clinical quality and Medicare compliance. This hybrid Bay Area role combines leadership, patient advocacy, and proactive care delivery.


Registered Nurse (RN) Case Manager Key Responsibilities

  • Supervise and support LPNs in care coordination and patient engagement.
  • Review and sign off on care plans and documentation.
  • Ensure compliance with Medicare PCM/CCM requirements through quality reviews.
  • Serve as liaison between physicians, patients, and care teams.
  • Mentor staff and collaborate on workflow and quality improvements.
  • Maintain accurate, time-based documentation for compliant billing.


Registered Nurse (RN) Case Manager Qualifications

  • Active, unrestricted RN license in California (multi-state a plus).
  • 2+ years’ experience in chronic disease management, neurology, or care coordination.
  • Experience supervising or mentoring clinical staff.
  • Proficient with EMRs and digital health tools.
  • Strong communication, leadership, and organizational skills.


Preferred

  • Knowledge of Medicare PCM/CCM billing.
  • Familiarity with Alzheimer’s, Parkinson’s, or dementia care.
  • Background in telehealth, hybrid-care models, or outpatient neurology.
