Registered Nurse – RN Case Manager
Professional Case Management provides in‑home care to former Nuclear Weapons Workers suffering from chronic and terminal illnesses caused by toxic exposure while working for the government. RN‑Case Managers conduct weekly nursing visits lasting 1–2 hours, performing physical assessments, ensuring home safety, filling medication plans, and coordinating caregivers.

Flexible schedule, $42/hour (including hours for drive time) and comprehensive benefits.

Responsibilities

Provide direct care and case management of assigned clients.

Assist and collaborate with the regional director and other personnel to identify and correct issues and improve services.

Plan, implement, and evaluate client care; coordinate client care conferences as needed.

Serve as a local on‑site clinical resource and support client home care needs.

Assist staffing coordinators regarding staffing and scheduling of personnel within scope of practice.

Adhere to nursing delegation guidelines and Agency policies.

Review clinical documentation for accuracy and completeness.

Participate in Agency performance improvement initiatives, including quarterly medical record review.

Collect, document, and submit data on infections, occurrences, complaints, grievances, and performance improvement activities.

Conduct supervisory visits to facilitate problem resolution.

Qualifications

Graduate of a state‑approved school of professional registered nursing.

BSN preferred.

Current, unrestricted RN license in the state(s) of practice.

Minimum of two (2) years nursing experience, including one (1) year in home care or closely related field.

One (1) year of supervisory and/or case management experience preferred.

Current CPR certification.

Salary
Standard rate: $42.00 hourly plus shift differentials, where applicable.
