Home Telehealth Registered Nurse (HT RN)

The Home Telehealth RN (HT RN) coordinates care for our Veteran population to ensure care is timely, evidence-based, appropriate, of high quality, and cost effective. This position works under the Telehealth Registered Nurse Manager for the Associate Director for Patient Care Service Line. The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. The Home Telehealth RN (HT RN) works closely with the PACT teams, other services lines, other healthcare staff, internal and external services, and community agencies. Other position criteria includes, but not limited to:

  • Provides initial and ongoing assessment of patients to identify needs, care goals and appropriate resources necessary for care management.
  • Provides leadership in application of the nursing process and identifies resources and critical factors for achieving desired outcomes for discharge, post hospitalization and health maintenance and improvement.
  • Provides advanced assessment and proactive care coordination to patients with one or more chronic conditions.
  • Works with health care team, patient and family to develop a customized plan of care short and long term goals.
  • Assess all data received from patients and contacts patients with high risk responses and/or significant changes in condition.
  • Uses established methods for tracking patients' progress, evaluating effectiveness of care and maintains appropriate documentation of each patient's care and progress within the plan.
  • Accurately documents interventions taken in response to patient care and completes encounters for care coordination/telehealth workload credit.
  • Assess patients for use of appropriate telehealth devices and follows procedures for ordering equipment.
  • Recognizes impact of age-specific care needs and incorporates this into assessment process.

Work Schedule: Monday through Friday 0800-1630

Telework: ADHOC - May be authorized (Employees who Telework on an occasional, episodic, or short-term, and have completed required telework training and telework agreement). This is not a virtual position.

Requirements Conditions of Employment: U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Qualifications Basic Requirements:

  • English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program.
  • In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case basis).

Preferred Experience: BSN or higher preferred 3 years successful nursing practice. Prior experience with program implementation, organization performance improvement processes and care management to include triage Ability to communicate and work effectively with a variety of disciplines to include providers, nurses, social workers and other staff
