The Home Telehealth RN (HT RN) coordinates care for our Veteran population to ensure care is timely, evidence-based, appropriate, of high quality, and cost effective. This position works under the Telehealth Registered Nurse Manager for the Associate Director for Patient Care Service Line. The Home Telehealth RN (HT RN) works closely with the PACT teams, other services lines, other healthcare staff, internal and external services, and community agencies. Other position criteria includes, but not limited to:
Work Schedule: Monday through Friday 0800-1630
Telework: ADHOC - May be authorized (Employees who Telework on an occasional, episodic, or short-term, and have completed required telework training and telework agreement). This is not a virtual position.
Relocation/Recruitment Incentives: Not Authorized

Qualifications Basic Requirements:
Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case basis).
Preferred Experience: BSN or higher preferred 3 years successful nursing practice. Prior experience with program implementation, organization performance improvement processes and care management to include triage Ability to communicate and work effectively with a variety of disciplines to include providers, nurses, social workers and other staff
