A-Line Staffing is now hiring a Full-Time, Registered Nurse or Licensed Clinical Social Worker Care Manager, in Rocky Mount, NC!

The Registered Nurse Care Manager supports members with Long-Term Services & Supports (LTSS) needs to ensure coordinated, high-quality, and cost-effective care. This role develops individualized service care plans, conducts member assessments, coordinates referrals, and ensures services are delivered without gaps. The care manager will work directly with members, caregivers, physicians, and community partners to support long-term care goals.

This is a field-based, member-facing role with required in-home visits.

Registered Nurse – Field Case Manager Compensation

• The pay for this position is $38.00/hr
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Registered Nurse – Field Case Manager Highlights

• This position is a contract assignment with potential for permanent hire based on attendance, performance, and business needs
• The required availability for this position Monday-Friday 8am-5pm EST

Registered Nurse – Field Case Manager Responsibilities

·         Manage an assigned caseload of LTSS members.

·         Conduct in-person assessments to gather clinical, functional, and environmental information.

·         Develop and maintain individualized care plans.

·         Monitor delivery and quality of services; identify and address care gaps.

·         Coordinate referrals and communicate with providers, community services, and caregivers.

·         Educate members and families on available benefits and supports.

·         Collaborate with discharge planners and care teams to support transitions of care.

·         Assist members with grievances, appeals, and care barriers.

·         Maintain accurate documentation within electronic medical record systems.

Registered Nurse – Field Case Manager Requirements

·         Licensure:

• • RN Licensure OR LCSW

·         Experience:

• • 2+ years of physical health care management experience required
  • 4–6+ years preferred

·         Strong background in hospital case management, discharge planning, home health, or long-term care.

·         Valid driver’s license and ability to travel within assigned region.

·         Proficiency with Microsoft Office & EMR/EHR systems.
