This position functions within the nursing department and is responsible for coordinating the delivery of appropriate and timely care for patients, promoting quality and cost-effective health care outcomes.
Key Responsibilities
Active management of SEARHC patients.
Collaborates with the medical provider and interdisciplinary team to establish individualized medical goals that maximize patient healthcare outcomes.
Coordinates multi-disciplinary care conferences for high-risk or complex patients referred to the case management process.
Manages panel acute and chronic care needs and health maintenance by reviewing EHR reminders and triggers, and providing outreach and education to patients.
Participates in case management reviews with a multi-disciplinary team of providers.
Works with the team as an advocate for patients/families at the service-delivery level, fostering decision making, independence, self-care and growth.
Uses problem-solving and anticipatory skills to address patient needs promptly and efficiently.
Facilitates referral coordination within SEARHC and outside facilities; tracks progress and outcomes, provides status reports, and identifies delays to remedy cases and improve processes.
Performs staff nursing duties as assigned, including assessment, documentation, education and administering medications/therapies.
Maintains annual competencies including safety and infection control; maintains a clean and safe working environment and adheres to infection control policies.
Communicates and works collaboratively with team members across departments; participates in department and clinic improvement efforts.
Active member of the clinical team and a leader among the pod, nursing staff and unit coordinators.
Participates in ongoing development of the case management role in primary care.
Participates in triage and responds to emergency situations as an RN.
Performs other duties as assigned, including special projects to provide comprehensive health services.
Education, Certifications, and Licenses
Graduate of an accredited school of nursing; Bachelor’s degree preferred.
Current full, unrestricted Registered Nursing license in Alaska or another U.S. state.
Direct Hire must have applied for an Alaska nursing license before start date.
Agency staff must have an active Alaska license.
Basic Life Support (BLS) certification preferred.
National Case Management certification preferred.
High school diploma or equivalent required.
Clinical Competency required within 3 months of hire and every three years.
Experience
At least 1 year of general nursing experience with at least 6 months in a SEARHC outpatient clinic, or at least 2 years of general nursing experience with at least 1 year in an ambulatory setting, or 1 year case management experience.
Knowledge and Skills
Knowledge of SEARHC services and those of other tribal health organizations in Alaska.
Knowledge of the nursing process and applying it in clinical settings.
Customer service principles; safety and infection control principles.
Assessment, anticipation of needs, data collection; use of computers and medical equipment; oral and written communications.
Abilities
Multi-task and work independently in a fast-paced environment.
Maintain compliance with annual competencies.
Problem-solve and use conflict resolution to collaborate with staff.
Anticipate needs and seek help when appropriate.
Work within a fast-paced clinical setting; move, stand, bend as required.
Follow directions and perform multiple tasks; apply evidence-based practices in a multi-disciplinary setting.
Physical Demands
Regularly able to talk or hear; stand, walk, sit; use hands; reach with arms.
Occasionally climb or balance; stoop, kneel, crouch or crawl; able to lift/move 50 lbs.
Certification
Basic Life Support (BLS) – American Heart Association.
