The Registered Nurse (RN) at Elmwood Healthcare - Home Health will be responsible for delivering comprehensive nursing care to patients in their homes. You will perform a range of clinical tasks, coordinate care with other healthcare professionals, and provide education and support to patients and their families.

Key Responsibilities:
Patient Care: Provide direct nursing care, including administering medications, wound care, IV therapy, and other treatments as prescribed by the physician.
Assessment and Evaluation: Conduct comprehensive assessments of patients' health status, including vital signs, medical history, and physical examinations.
Care Coordination: Collaborate with physicians, therapists, and other healthcare professionals to coordinate care and ensure a cohesive approach to patient management.
Patient and Family Education: Educate patients and their families on disease management, medication administration, self-care techniques, and lifestyle modifications.
Documentation: Maintain accurate, detailed, and timely documentation of patient care, including progress notes, treatment plans, and any changes in patients' conditions.
Health and Safety Compliance: Adhere to infection control practices, safety protocols, and all relevant regulations to ensure a safe care environment.
Emotional Support: Provide compassionate support to patients and their families, addressing concerns and offering comfort and reassurance as needed.

Infusion Therapies (Excluding IV Push)
Drain Care and Management
Catheter Care and Management
Ostomy Care and Management
VAD Management and VAD Site Care (Excluding Discontinuation)
Venipuncture and Blood Sample Collection via VAD
Qualifications Education: Associate's, Bachelor's Master’s or Doctoral degree in Nursing from an accredited program.
Licensure: Current and valid license to practice Registered Nursing (RN) in the Commonwealth of Massachusetts.
Experience: Minimum of one (1) year of experience as a Registered Nurse (RN), preferably in a home healthcare setting.
Skills: Strong clinical assessment and therapeutic skills, excellent communication and interpersonal skills, ability to work independently and manage time effectively.
Certifications: CPR Certification with Basic Life Support (BLS) or Advanced Life Support (ALS) is required.
Other Requirements: Valid driver’s license, reliable transportation, and willingness to travel to patients' homes within the designated service area.
