Overview Registered Nurse Case Manager PRN at Medical City Fort Worth. This role covers weekday and weekend shifts as needed, with typical hours 8:00am–4:30pm. Orientation/training will occur during weekdays for the first weeks.
What you will do Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients and families, and assesses available resources.
Reassesses clinical condition as indicated; develops strategies to mitigate readmission risk, including education on accessing healthcare resources, preventive education, and community resources.
Coordinates the plan of care and drives the discharge plan by collaborating with the multidisciplinary team and the patient’s physician to facilitate a successful care transition.
Partner with Social Services to ensure appropriate post-acute medical needs and level of care; refer to Social Services when psychosocial risk factors are identified.
Involves patient and family in identifying needs and setting mutual, realistic goals.
Evaluates progression of care using evidence-based tools (InterQual) and escalates issues through the chain of command.
Refers patients to third-party payer programs, disease and case management programs as needed.
Facilitates patient throughput with a focus on quality, efficiency, and safe discharge/transition.
Documents recommendations, discharge plans, care coordination interventions, and case management activities for the health care team.
Aligns patient needs with available resources for safe discharge/transition.
Communicates professionally with physicians, patients/families, hospital staff, and outside agencies; advocates for patient rights and culturally sensitive care.
Identifies and provides for the needs of under-resourced patients, including patient education, assistance programs, and community resources.
Maintains knowledge of regulatory requirements, ethics and compliance policies, and quality initiatives.
Responsibilities in summary Promote patient-centered care and coordinate the plan of care throughout the patient stay.
Ensure appropriate resource utilization and safe discharge planning.
Collaborate with multidisciplinary teams to support quality outcomes and patient throughput.
Qualifications Registered Nurse with current TX state license required.
Associate Degree in Nursing or Nursing Diploma required; Bachelor's Degree in Nursing preferred.
3+ years of clinical hospital nursing experience required OR 2+ years of case management experience required.
Certification in case management preferred; InterQual experience preferred.
