Position Overview
Seeking Full-Time RNCM for our Hospice Agency. For our Orange County region including MISSION VIEJO Hospice Experience preferred. We are hiring a Registered Nurse Case Manager for our growing Hospice to join our vibrant team!
Responsibilities
You will be responsible for the assessment, diagnosis, and treatment of assigned patients. Hospice experience is desired. You will initiate and coordinate the hospice care plan, supervise CHHAs and LVNs and provide skilled nursing hospice and palliative care services to patients. The RN works cooperatively with the attending physician, Hospice Medical Director, and other members of the interdisciplinary Hospice team. The RN regularly reviews and updates the plan of care, performs prescribed medical treatments (including pain management and symptom control), conducts assessments and evaluations and provides education and supportive care to patient and caregivers. Must be able to utilize computers and be comfortable with EMR.
Qualifications
Graduate of an accredited school of nursing.
Current RN license within the State.
BSN, Certification in Hospice and Palliative Nursing desired.
Valid driver's license, auto insurance, reliable/well-maintained transportation.
