The individuals that live in our homes have a variety of intellectual and developmental disabilities as well as physical impairments. Our homes are licensed through the California Department of Public Health and are classified as ICF/DD-N (Intermediate Care Facility/Developmentally Disabled - Nursing) and ICF/DD-H (Intermediate Care Facility/Developmentally Disabled - Habilitative) facilities.
Our clients come and go throughout the day and weekend to attend Day Programs, Work Programs, and visit family and friends; some are more independent than others in their outings and we support them whenever we can so they can have free access to their community and community events. We also encourage fully staffed whole house outings to places like the beach, Sonoma County Fair, baseball games, dances, concerts, and the like. If our staff is having fun then so are our clients!
The Registered Nurse's major responsibility is the promotion and maintenance of client health through teaching, counseling, and appropriate preventative and habilitative measures. The Registered Nurse Case Manager serves as a member of each client's individual service plan team and provides direct input toward the development of the clients' individual service plan.
Part-Time 10-15 hours a week Hybrid . Must reside within 25 miles of Santa Rosa, CA
Registered Nurse Case Manager Duties and Tasks:
Develop, implement, sustain, and coordinate healthcare services.
Participate as an active member of the interdisciplinary team (IDT).
Plan and implement nursing and health-related education programs for direct care staff.
Act as a clinical resource and staff support to the House Managers, QIDPs, IDT, and direct care staff.
Understand regulatory channels for ICFs and provide client care within the confines of said regulations.
Perform as a nursing consultant to the ICF and follow up on all medical recommendations, documenting, and ISP meetings.
Be responsible for the oversight of one or two homes with a total of 6-12 clients.
Participate in the monthly House Meetings and periodic company-wide in-service meetings.
Requirements:
Registered Nurse Case Manager Job Requirements:
CA RN License
Prefer two years of experience in a community health service, long-term care, and/or serving developmentally disabled or a related population.
Adaptability: The healthcare environment can be unpredictable, so the ability to adapt to changes and new situations is valuable.
Teamwork: Collaborating with other healthcare professionals and support staff is vital for delivering comprehensive care to patients.
Communication: Effective communication skills are essential for interacting with patients, their families, and other healthcare professionals.
Must reside within 25 miles of Santa Rosa, CA
