Attractive Pay

$80,000-$85,000

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.

Make referrals to other hospice care disciplines as needed or document rationale for not doing so.

Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness, and in accordance with care center policies.

Record pain/symptom management changes/outcomes as appropriate.

Teach patients, families, and caregivers appropriate patient care techniques.

Regularly communicate with clinical manager and care team.

Perform on‑call responsibilities and on‑call services to patients/families as assigned.

Perform other duties as assigned.

Qualifications

One year of nursing experience.

Current RN license specific to the state(s) you are assigned to work.

Current CPR certification.

Valid driver's license, reliable transportation, and liability insurance.
