Pay: $60 - $67 per hour

Agape Hospice NW is seeking experienced Registered Nurse Case Managers for the McMinnville to Salem area to provide exceptional, patient-centered care to individuals and families. Job Overview:
As an RN Case Manager, you will plan, organize, and manage hospice care for patients in their homes and communities. You’ll leverage local resources to meet patient and family needs, providing care and educational support. Your expertise in nursing and leadership will help guide a holistic care approach.

Key Responsibilities ( This is a summary, not a complete list):

  • Patient Care: Conduct comprehensive assessments, create and implement care plans, administer medications and treatments, and provide health education.
  • Communication: Maintain accurate clinical notes, coordinate care with physicians and interdisciplinary team members, and support patients and families with self-care education.
  • Collaboration: Work with the hospice team to address emotional and psychological needs and provide continuity of care.
  • Additional Duties: Participate in on-call shifts (on-call /beeper pay), supervise ancillary staff, and engage in professional development and quality improvement activities.

Qualifications:

  • Oregon RN license
  • Hospice experience preferred
  • Strong communication and leadership skills
  • Valid driver’s license and reliable transportation
