Role Summary
As an RN Home Health Case Manager, you will create, implement, and execute individualized care plans to promote wellness and prevent rehospitalization for patients in their homes. You will provide education and oversight for wound care, blood draws, tube feedings, tracheostomy care, Foley catheter care, CHF management, and medication administration.
Responsibilities
Perform full assessment of patient and family at initial visit.
Develop advanced plan of care and rehabilitation goals based on assessment data.
Collaborate with patients and families one‑on‑one to achieve improvement and stability.
Case‑manage patient’s care episode with interdisciplinary clinical team.
Prepare clinical notes, orders, and charting in a compliant time frame.
Collaborate with physicians and agency staff to deliver the best care possible.
Initiate and administer IV medication infusions safely in accordance with established protocols.
Qualifications
Current and unrestricted California RN license.
BLS/CPR certification.
Valid California driver’s license.
High level of clinical knowledge in home health care (high acuity experience preferred).
Experience working with interdisciplinary teams and case management fundamentals.
Experience with IV medication infusions preferred.
Self‑directed, empathetic, critical thinker with outstanding interpersonal skills and leadership ability.
Organized and ability to complete a computer literacy and competency assessment prior to interview.

Compensation
Base pay: $130,000.00 – $160,000.00 per year plus annual bonus (approx. $10K). Paid quarterly.
