Position Summary
The RN Case Manager plans, organizes, and provides home health care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Responsibilities
- Conduct assessments and complete client visits
- Coordinate care alongside therapies and clients/families, and coordinate, supervise, monitor, and develop care
- Communicate with physicians
- Perform teaching to clients and families
- Complete complex nursing tasks within the home setting
Qualifications
- Possess a valid RN License from the appropriate state. This shall include required CEUs
- Minimum 2 years of nursing experience
- Oasis experience (Preferred will provide training)
- Home Health experience required
