Registered Nurse - Case Manager

Pay Range
$55.00/hr - $60.00/hr

Responsibilities

Perform care coordination and discharge planning in an acute hospital setting.

Conduct needs assessments and order Durable Medical Equipment (DME) as required.

Develop and implement plans of care in compliance with regulatory standards.

Ensure compliance with CMS guidelines, CPT coding, DRG, and HIPAA regulations.

Work with interdisciplinary teams to manage patient care effectively.

Manage a daily case load of up to 25 patients, focusing on adults and geriatrics.

Provide weekend coverage every other weekend, typically one full weekend per 4 weeks.

Skills

Proficiency in Epic charting system.

Experience in acute hospital case management and utilization review.

Ability to determine medical necessity using evidence‑based guidelines.

Familiarity with HMO and IPA operations.

Skilled in computerized documentation.

Experience

Minimum of 2 years of acute medical care management experience with a focus on care coordination and discharge planning.

Travel experience is required, though first‑time travelers will be considered.

Experience in ICU, MS, SDU/PCU/IMC/Obs settings is advantageous.
