Overview
Position: Registered Nurse Case Manager
Shift: Day 5x8-Hour (08:00 - 16:30)
Location: Eureka, CA 95501
Start date: 12/01/2025
Pay: $70-$75 per hour
Responsibilities
Responsible for the quality and resource management of all patients that are admitted to the facility from the point of admission and across the continuum of health care management.
Work on behalf of the advocate, promoting cost containment and demonstrate leadership to integrate the health care providers to achieve a perceived seamless delivery of care.
Use a holistic and comprehensive admission and concurrent review of the medical record for medical necessity, intensity of service, and severity of illness to facilitate quality, clinical and cost-effective outcomes.
Weekend requirements: Required to work every other weekend.
Must have acute care experience.
Case Management – Start date: ASAP.
Years of experience required: 2 years.
First-timers accepted: Yes.
Weekends required: EOW.
Floating: Will travel to similar units as needed based on demand.
RTO restrictions: Please include all RTO at the time of submittal.
Charting: Epic.
Qualifications
EXP: -2 year acute Hospital Case Management experience performing care coordination and discharge planning.
2 years of relevant experience required.
