Guaranteed Hours: 8 and 10 hr. shifts - 40 hrs. in one workweek. 3, 12 hr shifts - 36 hrs. in one workweek. 4, 12 hr. shifts - 48 hrs. in one workweek.
Special Requirements: This is an RN Case Manager position on a discharge planning team. Discharge assessment and coordination of discharge plan are the primary responsibilities. Acute Hospital, ICU, MS, SDU/PCU/IMC/Obs, Care coordination, Discharge Planning, CMS: Centers for Medicare and Medicaid Services.
Unit Information: Units include ICU, PCU, Medical, Oncology, and Surgical. Processes these nurses will be involved with include Discharge Planning Assessment and coordination of acquisition of discharge resources like Skilled Nursing Facility Placement, Home Health Care Arrangements, Durable Medical Equipment, etc... This is bedside work at times, but not traditional direct patient care.
