Overview
Join our Community Hospice team as a Case Manager Registered Nurse to build lasting connections with patients and their families. We provide compassionate, expert care that allows individuals to remain comfortably at home. As a key member of our interdisciplinary team, you will deliver skilled nursing care, educate patients and families, and support decisions that honor their wishes.
Responsibilities
Deliver skilled nursing care in the home setting as part of an interdisciplinary team.
Educate patients and families about care plans, treatments, and end-of-life preferences.
Create a supportive environment that honors patient wishes and promotes quality of life at home.
Requirements
Current RN licensure recognized by the State of Vermont.
2 years' experience in home health, home hospice, or med surg, or a combination of these settings.
On-call
On-call is not typically required; it is variable depending on program needs.
