Registered Nurse Case Manager position supports the physician and interdisciplinary team in facilitating patient care, aiming to enhance clinical outcomes, patient satisfaction, and manage cost of care.
Base pay range: $73,860.80/yr - $98,234.86/yr

Responsibilities
Facilitate collaborative management of patient care across the continuum.
Intervene to remove barriers to timely and efficient care delivery and reimbursement.
Apply process improvement methodologies to evaluate outcomes of care.
Coordinate communication with the healthcare team, patients, families, and post-acute care services.
Screen and prevent readmission.
Qualifications
Degrees: Associates
3 years hospital clinical experience with a minimum of 1-3 years of hospital or payor case management experience preferred.
Excellent interpersonal communication and negotiation skills.
Strong analytical, data management, and computer skills.
Current working knowledge of discharge planning, utilization management, case management, and performance improvement preferred.
Understanding of pre-acute and post-acute venues of care and post-community resources preferred.
Strong organizational and time management skills, evidenced by capacity to prioritize multiple tasks and role components.
Ability to work independently and exercise sound judgment in interactions with the healthcare team and patients/families.
