Registered Nurse Case Manager – Home Health and Hospice

Overview
The Registered Nurse Case Manager provides comprehensive care coordination and nursing services for patients in a home health and hospice setting. This role involves managing individualized care plans, collaborating with interdisciplinary teams, and supporting patients and families through compassionate and skilled nursing care. The nurse will ensure continuity of care, patient safety, and adherence to best practices across all assigned cases.
Key Responsibilities
Provide case management and skilled nursing care to patients in home and hospice settings
Coordinate with physicians, therapists, social workers, and other care team members to ensure effective care delivery
Monitor patient conditions, evaluate care needs, and update care plans accordingly
Educate patients and families on disease management, medications, and end-of-life care
Document all clinical information accurately in accordance with regulatory and organizational standards
Participate in interdisciplinary team meetings, case reviews, and quality improvement initiatives
Manage time effectively to balance clinical visits, documentation, and communication
Respond promptly to patient needs and emergencies as required
Perform other nursing duties as assigned
Schedule and Shift Details
Monday through Friday, 8 AM to 5 PM, with variable start and end times
On-call rotation approximately five shifts per month
No regular weekend or holiday requirements unless assigned during rotation
Travel
Local travel required across multiple counties for home visits and case management
Qualifications Required
Active Registered Nurse (RN) license in Illinois (IL)
Minimum 1 year of hospice experience
Minimum 1 year of management or leadership experience preferred
Experience in home health or community-based care
Current certifications: Basic Life Support (AHA BLS) and Advanced Cardiac Life Support (AHA ACLS)
High School Diploma or GED or higher education in nursing
Reliable transportation and valid driver’s license
Preferred
Prior experience in care coordination or case management
Knowledge of EMR systems and electronic documentation
