Western Tidewater CSB is seeking a Registered Nurse/Case Manager – CCSU Clinic.

Mental Health And ID/DD Experience Strongly Preferred

Responsibilities and Duties

Partner with clients, providers, caregivers and interdisciplinary teams to meet health care goals and ensure continuity of care.

Coordinate Discharge/Transition Planning with clients and their receiving caregiver.

Assess client needs and provide service linkage and coordination in alignment with the Individualized Service Plan to promote continued community stability and wellness.

Develop, implement, coordinate, and monitor the comprehensive Individual Service Plan (ISP) for each client.

Responsible for consumer assessment, service linkage, coordination, development, and implementation of Individual Service Plans (ISPs).

Provide follow-up instruction, education, and consultation to clients and their families regarding the client’s wellness needs.

Monitor service delivery through regular contact with consumers, family members, service providers, and periodic site/home visits.

Manage consumer records according to all Licensure, Medicaid, and state/federal standards.

Nursing & Clinical Care

Implement nursing and care management services across the continuum of care in the CCSU Clinic.

Serve as nursing staff working in the Bridges Home to provide nursing support for children while assisting CCSU staff.

Collaborate with the CCSU team to support crisis stabilization goals.

Ensure medications are received, stored, and distributed safely and in compliance with regulations.

Conduct ongoing physical and mental health assessments; monitor consumer safety.

Provide guidance to behavioral staff on medical treatment needs and assessments.

Educate and monitor consumers and families regarding medication compliance and potential side effects.

Coordinate care with physicians, nurse practitioners, and internal/external providers as needed.

Qualifications and Skills

Licensure as a Registered Nurse in the Commonwealth of Virginia.

QMHP or QIDP preferred.

Minimum one (1) year of clinical experience in nursing and at least one (1) year providing mental health services to adults with serious mental illness.

BA/BS in a related Human Services field (Psychology, Social Work, or related discipline) preferred.

Proficiency in Microsoft Office; experience with electronic medical records and basic knowledge of Medicaid/Medicare preferred.

Valid Driver’s License with an acceptable DMV record at the time of hire and maintained throughout employment.
