Overview

Provide case management services as a Registered Nurse in an acute hospital setting.

Collaborate with interdisciplinary teams to ensure safe and timely patient discharges.

Integrate utilization management and case management strategies effectively.

Navigate payer environments and managed care protocols efficiently.

Maintain compliance with CMS standards and hospital policies.

Utilize critical thinking and problem-solving skills in a fast-paced environment.

Support the progression of care by applying clinical guidelines.

Demonstrate professional communication and interpersonal skills.

Key Responsibilities & Duties

Coordinate patient care plans and ensure effective implementation.

Collaborate with healthcare professionals to optimize patient outcomes.

Perform utilization reviews and discharge planning activities.

Adhere to hospital policies and regulatory requirements.

Provide patient education and support during discharge processes.

Maintain accurate and timely documentation of patient care activities.

Ensure seamless communication between patients, families, and care teams.

Monitor patient progress and adjust care plans as needed.

Job Requirements

Active Registered Nurse (RN) license in applicable state(s).

BLS certification required for hospital-based roles.

Associate of Applied Science (AAS) degree in Nursing.

Minimum 2 years of acute hospital nursing experience.

Preferred certifications: CCM, ACM-RN, or Utilization Management.

Experience in Care Coordination or Case Management in acute hospital settings.

Knowledge of managed care, utilization review, and discharge planning.

Strong critical thinking and problem-solving abilities.
