About the role: The Registered Nurse Case Manager plans, organizes, and directs hospice care and is experienced in nursing, with an emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Job Responsibilities: Completes an initial, comprehensive assessment of the patient and family to determine hospice needs, including a complete physical assessment and history of current and previous illnesses.
Provides professional nursing care utilizing all elements of the nursing process.
Assesses and evaluates patient status by writing and initiating a plan of care, regularly re-evaluating patient and family/caregiver needs, and revising the plan of care as necessary.
Initiates the plan of care and makes revisions as patient status changes.
Uses health assessment data to determine nursing diagnoses and develops care plans with patient and family involvement.
Initiates preventive and rehabilitative nursing procedures and administers medications and treatments as prescribed.
Counsels the patient and family in meeting nursing and related needs and provides health care instructions as appropriate.
Assists the patient with activities of daily living and supports self-sufficiency and comfort care goals.
Acts as Case Manager when assigned by Clinical Supervisor and coordinates patient care for the assigned caseload.
Communication Completes, maintains, and submits accurate clinical notes regarding the patients condition and care given; records pain/symptom management changes/outcomes as appropriate.
Communicates with the physician regarding the patients needs and reports changes in condition; obtains physician orders as required.
Coordinates with community health-related persons to align the care plan; teaches patient and family self-care techniques as appropriate.
Provides medications, diet, and other instructions as ordered by the physician; works with the interdisciplinary team to ensure safe care.
Maintains a safe environment for the patient and supports continuity of care with the family/caregiver and hospice interdisciplinary group.
Attends interdisciplinary group meetings.
Additional Duties Participates in on-call duties as defined by policy.
Ensures arrangements for equipment and services are available; supervises ancillary personnel as required.
Maintains and upgrades professional knowledge through continuing education and in-service classes.
Performs assigned case work and participates in quality assessment and performance improvement activities.
Job Qualifications: Graduate of an accredited school of nursing; current licensure in the state; CPR certification.
Bachelors degree preferred, with hospice care experience; one to two years of recent acute care and home care experience preferred.
Management experience is not required; responsible for supervising hospice aides.
Excellent observation, communication, problem-solving, and nursing skills; ability to perform required nursing competencies.
Ability to stand/walk for extended periods; capable of lifting, positioning, and transferring patients and lifting supplies; good visual and auditory abilities.
Must have reliable transportation and a valid drivers license; vehicle must be insured and in good working order.
