Overview
Ovation Hospice, is currently seeking a highly skilled and compassionate Registered Nurse/ Case Manager in Salt Lake City, UT, with a focus on geriatrics, hospice, and palliative care to join our Hospice team. As the Registered Nursing/Case Manager with Ovation, you will be responsible for planning, coordinate and provide hospice care in accordance with the patient's plan of care, and ensuring the highest standards of care are upheld. Your expertise will be instrumental in providing exceptional nursing care and ensuring the well-being of our patients.

Responsibilities

Nursing services, treatments and preventative procedures requiring substantial specialized skill and ordered by the physician.

Be competent in knowledge about different levels of care and when it’s appropriate to offer them.

Utilize the nursing process to provide professional nursing care.

Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis.

Perform evaluations for hospice care along with informational visits as needed.

Evaluate the effectiveness of her nursing service to the individual and family.

Collaborate with interdisciplinary teams to develop and review individualized patient care plans

Foster a culture of teamwork, respect, and collaboration among the nursing staff and interdisciplinary team members

Participate in community outreach initiatives, education programs, and professional development activities

Collaborate with other department leaders to optimize organizational effectiveness and efficiency

Qualifications

Registered Nurse (RN) license in good standing

Bachelor's degree in Nursing

Minimum of 1 years of RN experience, Hospice experience preferred.

Excellent clinical assessment and critical thinking skills

Strong organizational and problem-solving abilities

Effective communication and interpersonal skills, with the ability to build rapport with patients, families, and interdisciplinary team members

Proficiency in using electronic medical record (EMR) systems and other relevant healthcare software

Compassionate, empathetic, and dedicated to providing quality end-of-life care
