The Registered Nurse Case Manager (RNCM) is experienced in nursing, and leads each patient's hospice care team, helping to create and manage their specific Plan of Care in collaboration with the Hospice Medical Director, patient, family and/or facility staff. The RN Case Manager provides day-to-day patient care, manages treatment, administers medications, maintains patient records and directs other members of the patient care team to meet the changing needs of the hospice patient, with a focus on providing comfort and enhanced quality of life.

Must have a current Registered Nurse (RN) license in state(s) of practice. Nurses holding a multistate license can practice in other states without obtaining additional licenses while maintaining their Registered Nurse (RN) license in their primary state of residence (PSOR). Minnesota, Michigan, and Illinois are not in the Nursing Licensure Compact. Graduate of a professional school of nursing and is currently licensed, certified or registered in the state(s) of practice. Ability to pass DHS background, consumer report, and drug test. Valid State Driver's License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required. Preferred: 1+ years of recent experience in hospice/home care preferred. 2+ years of Nursing experience, preferred.

The starting salary range for this role is $75,000-$101,000 annual base salary. An employee's actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.
