Description
The Nurse Care Manager utilizes expert clinical skills and a disease‑specific knowledge base to assist identified patients through the continuum of care in an effort to provide appropriate, quality care in a cost‑effective manner. The Nurse Care Manager conducts daily assessments to identify high‑risk patients and manages their care during the acute care episode.
This position supports organizational goals by providing quality customer service, participating in performance improvement efforts, and demonstrating a commitment to teamwork and cooperation.
Qualifications
Education: Graduate of an accredited school of nursing required. Bachelor’s in Nursing preferred.
License/Certification: Current licensure as a Registered Nurse in the State of New Jersey and BLSHCP Certification required (within 6 months of hire or transfer). For some specialties, a Master’s degree in Nursing may be required, with an advanced practice nurse certification.
Experience: 5–7 years of clinical experience required. Prior case management/insurance management experience preferred. Excellent interpersonal, written communication skills, computer literacy, analytical and delegation skills required. Proficiency in Clinical Applications preferred at the time of hire; incumbents will receive training and must acquire required skill within 30–60 days.

Work Environment
Potential for exposure to the hazards and risks of the hospital environment, including blood‑borne pathogens, infectious disease, hazardous substances, and potential injury. Reporting Relationship
The Nurse Case Manager at Mainland reports to the Case Management Divisional Manager, and the Nurse Case Manager at City reports to the Case Management Director.
