Base pay range
$55.00/hr - $60.00/hr

Duties

Assess patients and coordinate care effectively.

Plan and participate in discharge planning and utilization review.

Organize and participate in family meetings.

Develop and implement treatment plans.

Work with HMOs, IPAs, and other healthcare organizations.

Manage adoption/foster care, drug/alcohol abuse treatment options, and care for patients experiencing homelessness or mental health crises.

Arrange DME, SNF, and ambulance transport independently.

Utilize computerized documentation systems.

Plan of Care and Prior Authorizations

Knowledge of CMS, Department of Health, DRG, and The Joint Commission standards

Experience

Minimum of 2 years in Acute Care Case Management/Discharge Planning

Experience with a patient ratio of 1:25

Proficiency in Epic charting system

Travel experience is required; must be a seasoned traveler

Experience in Emergency Department, ICU, MS, and Surgical Services is beneficial

Must have hospital experience; behavioral health experience does not apply
