Patient Types/Common Diagnoses:
Travelers will care for patients in NICU, pediatric, ER, MedSurg, Tele, Oncology, and ICU. No L&D
Case Load:
As low as 15 patients and as high as 25-30, depending on staffing
Required License, National Certification, Certs:
BLS, BSN, State of WA RN license
Skills Required:
Experience with above patients, case management and discharge planning, strong assessment skills, ability to discharge homeless patients and discharge patients to the next level of care to SNF, IP, LTAC, Shelter, Assisted Living Facility, Medicare/Medicaid patients.

Case Management Social Workers
Case Management Assistant
CM Manager daily
Will be partnered with a CM Social Worker
Technology/Equipment:
EMR: EPIC experience preferred

RTO Approval: Ok to approve 7 days
Shift times: 0700-1730, four 10h days. 2 15 mins breaks and 1 30 min meal break.
