Registered Nurse Care Coordinator Oncology

This RN is an integral part of the Cancer Center team, providing professional nursing care in our state of the art Cancer Center. Assist throughout the center in a variety of ways to provide high quality care to our population of both hematology & oncology patients. Job duties may include all of the following: performing assessments, telephone triage, administering injections, infusion treatments or chemotherapy as assigned. Plans & coordinates care with the primary physicians & other disciplines. Work collaboratively with a designated physician to provide a team approach to holistic patient care. The team of physician and RN Care Coordinator will facilitate their patient's plan of care throughout their treatment. This approach will provide continuity for the patients and emphasize one-to-one nurse patient relationships along with safe, high-quality care. Reports to Manager/Director Oncology.

Requirements:

  • RN license NH or other Compact State
  • Prefer oncology experience minimum of 2 years and/or work in an ambulatory setting; OCN preferred
  • 1 to 3 years Directly related experience
  • Associate's Degree
  • Strong customer service and analytical skills

Responsibilities:

  • Work as a team member with the physician to monitor and manage patient care for their designated patient caseload.
  • Collaborate with schedulers, infusion nursing staff, social work, medical assistants, office staff and other multidisciplinary departments.
  • Administer injectable therapies as ordered.
  • Triage calls from patients and outside medical offices. Coordinate sick visits as needed.
  • Follow up and triage of all needs designated by the provider within 1-2 business days.
  • Be aware of key issues and changes with your team of patients.
  • RN Symptom management visits to review lab results for active treatment patients.
  • Oral medication management including refill requests, prior authorizations, and education.
  • Participate in weekly provider, scheduler, NCC, rounds, palliative care rounds, clinics, tumor board as appropriate.
  • All other duties as assigned
