Registered Nurse - Care Coordinator

Job Opportunity At Saint Luke's East Hospital

Saint Luke's East Hospital in Lee's Summit, MO is seeking a Nurse Care Coordinator to join our team. Full time: Monday - Friday 8-4:30. This position will be on-site with 1 weekend rotation every 6 weeks required. The Care Coordinator is responsible for the big picture focus of care transitions through the continuum. An integral part of the multi-disciplinary team, the Care Coordinator is responsible for ensuring quality patient outcomes, monitoring resource utilization and mitigating obstacles to care delivery through interdisciplinary collaboration, communication and coordination. This includes developing and carrying out discharge plans and mitigation of barriers that impede care progression.

BSN Degree required within 3 years of start of position.

Responsibilities include:

• Assessing for needs and developing individualized discharge plan.
• Identifying additional services needed and making appropriate referrals.
• Leading and facilitating multi-disciplinary rounds. Helps to ensure clinical readiness for discharge.
• Collaborating with multidisciplinary team. Assists with throughput management and works to mitigate delays in care.
• In collaboration with attending physician, identifying appropriate length of stay and monitoring resource utilization.
• Working with patients, families and caregivers to ensure patient needs are met, and a timely and appropriate discharge plan is implemented.
• Maintaining appropriate documentation.

Job Requirements:

• Applicable Experience: 1 year Basic Life Support - American Heart Association or Red Cross, Registered Nurse - Various
• Bachelor's Degree - Science in Nursing
