Under general direction of the RN Patient Flow Manager and within the Lahey Clinic Professional Practice Model, MA Nurse Practice Act and ANA Standards of Practice and Code of Ethics, performs patient flow, transfer, and bed placement activities. Areas of responsibility include all in patient flow activities, electronic bed board operation, system coordination, quality improvement, professional growth and self-improvement, and other professional duties. In honoring our tertiary care mission to be the provider of choice for Cardiac patients local and beyond, we are expanding our team to include cardiology care coordinator nurses who will indirectly and directly work with our Cardiovascular Teams, patients and other acute and non-acute care facilities. This role will greatly contribute to our team's efforts in providing seamless care transitions for our cardiovascular patient population. This position allows for autonomy, flexibility and growth while serving our community beyond borders.

Essential Duties & Responsibilities including but not limited to:

  • Clinical Practice
    • Performs the patient flow, transfer and bed management process for Lahey Clinic
    • Works in compliance with Nursing Practice Guidelines
    • Performs bed flow functions and patient placement process
    • Optimizes patient flow throughout the hospital
    • Maintains and displays knowledge of current professional and clinical issues
  • System Coordination
    • Links departmental goals to organizations mission and goals
    • Works collaboratively to solve problems
    • Responds calmly and efficiently during crisis and/or emergency situations
    • Confronts and effectively deals with issues
    • Notifies appropriate colleagues of unsafe working conditions

Minimum Qualifications:

  • Education: Graduate of an accredited School of Nursing
  • Licensure, Certification, Registration: Current licensure to practice professional nursing in the Commonwealth of Massachusetts
  • Skills, Knowledge & Abilities: Clinical Skills, Technical Skills, Basic computer skills
  • Experience: A minimum of five years of accumulated in-patient nursing experience and two years leadership experience

Pay Range: $38.11 - $98.23
