Join Ambassador for Children and Families (ACAF) as an RRH (Rapid Re-Housing) Case Manager II and become a vital part of our mission to empower individuals and families. Case Managers may serve:

  • Individuals
  • Families
  • Domestic violence survivors
  • People with mental illness, substance use, physical disabilities, or chronic health needs
  • Those involved with DCFS or the justice system
  • Assignments may include Permanent Housing, Permanent Supportive Housing, Transitional Housing, and Rapid Re-Housing, Employment, and Income projects.

Services are trauma-informed, participant-centered, and strengths-based.

General Responsibilities:

  • Uphold and embody A.C.A.F’s Core Values in daily practice.
  • Maintain professionalism and high standards in all interactions.
  • Coordinate with agency departments to meet participant needs.
  • Attend staff meetings and participate in professional development.
  • Work both independently and collaboratively in a team setting.
  • Travel throughout Chicago for home visits and participant meetings.

Essential Duties:
Housing Support

  • Maintain regular monthly contact with assigned participants and landlords.
  • Prevent returns to homelessness through proactive engagement.
  • Assess referrals for eligibility and enroll qualified participants promptly.
  • Assist in apartment search, move-in support, and lease execution.
  • Maintain landlord relations and advocate for housing quality and retention.

Case Management

  • Identify and address barriers to housing stability.
  • Provide coaching, referrals, and connections to employment, healthcare, legal, and social supports.
  • Assist with budgeting, life skills, tenancy skills, and parenting support.
  • Develop and update individualized goal-based action plans.
  • Conduct regular home visits and follow-ups.
  • Ensure children are appropriately enrolled in school and early childhood services.

Documentation & Reporting

  • Maintain timely and accurate case notes and tracking in required systems.
  • Prepare payment requests and follow up on rent collection and arrears.
  • Collaborate with Finance to ensure lease compliance and payment processing.
  • Contribute feedback for program development and evaluation.

Program Compliance & Support

  • Ensure accurate data entry and reporting to internal and external stakeholders.
  • Support program audits and adhere to funding and regulatory requirements.
  • Assist with tenant feedback, surveys, and resident meetings.
  • Recommend service improvements based on participant needs and outcomes.

Supervision
This position may include leadership responsibilities over junior case management staff when assigned.

Job Qualifications:

  • High school diploma or equivalent required.
  • At least 2 years of case management experience with homeless or vulnerable populations.
  • Lived experience will be considered in place of formal education where appropriate.
  • Must have access to a personal vehicle, valid driver’s license, and insurance (for applicable roles).
  • Proficiency in Microsoft Office required;
  • Experience with HMIS (Homeless Management Information System) or electronic case recordkeeping required.
  • Familiarity with HUD guidelines, the Chicago CoC, and local housing systems preferred.
  • Excellent communication, critical thinking, and organizational skills.

Physical Demands

  • Frequent walking, standing, and traveling throughout Chicago.
  • Regular lifting of 10–20 lbs.
  • Ability to climb stairs and navigate housing units.
  • Reasonable accommodations provided as needed.

Work Environment

  • Hybrid work model (office, field, and remote work).
  • Standard business hours with occasional evenings or weekends required.
  • Office location: Ambassador for Children and Families main site.
