POSITION TITLE: Rapid Re-Housing Case Manager
The Rapid Re-Housing (RRH) Case Manager provides housing-focused case management services to high level acuity clients who are referred to the Long Beach Rapid Re-Housing program. The RRH Program is a time-limited subsidy program that provides limited financial assistance to clients to assist in maintaining their housing and provides tailored case management services to clients with the goal of clients achieving financial independence at the end of the program. The maximum time clients can be enrolled is up to 24 months based on assessments/funding. The RRH Case Manager will utilize trauma-informed, motivational interviewing techniques to assist clients in achieving their housing goals.
Responsibilities
Receiving referrals and coordinating intakes within an appropriate amount of time.
Processing and updating rental packets for each client in a timely manner.
Providing trauma-informed case management services to clients on their caseload. Case management services can include but are not limited to independent living skills, housing retention, safety planning, budgeting, scheduling appointments, benefit applications, etc.
Maintaining a caseload averaging 20 clients (can increase temporarily if programmatic needs require it).
Developing comprehensive, individualized Housing Retention Plans that address short- and long-term goals.
Developing budgets with clients at intake and every 90 days that will help them understand their responsibilities and how to reduce costs to help maintain their housing.
Working with clients to increase income so they can sustain their units independently.
Collaborating with property managers and other organizations to provide comprehensive services to clients.
Providing warm handoffs to outside organizations and ensuring the client is properly connected to services that provide quality of life improvements (substance use, mental health, medical benefits).
Conducting a minimum of one home visit a month with clients and follow-up contact to ensure clients are working on housing goals.
Maintaining accurate records of services to comply with contractual standards and HIPAA standards.
Maintain and execute confidential information in accordance with HIPAA standards.
Participate in regular check-ins with management and participate in program meetings as required.
Other duties as assigned by leadership.
Requirements
High School Diploma
One year experience working with People Experiencing Homelessness, preferably those living with mental illness and/or substance addictions or equivalent education and experience
Knowledge of case management techniques including trauma informed care, motivational interviewing, harm reduction, housing first model.
Ability to maintain professionalism and possess a high level of tolerance and understanding for individuals with urgent and multiple case management needs
HMIS experience is highly desirable
Be willing to work flexible hours which may include evenings or weekends on occasion
Must be able to travel to clients in either personal or company vehicle to meet the in-person requirements of the contract
Have reliable transportation, a valid driver’s license, auto insurance, and the ability to qualify for Helpline Youth Counseling’s insurance coverage
Clear and concise oral and written communication skills
Strong time management skills and the ability to coordinate multiple tasks with time-sensitive deadlines
