Radiation Oncology Ambulatory Clinic RN Care Coordinator
The position is located at Bloch Radiation Oncology Pavilion.

Position Summary
The Ambulatory Clinic RN Care Coordinator is a licensed registered professional nurse who provides ethical, evidence‑based, safe and compassionate nursing care. The RN focuses on patient safety and quality of nursing care by identifying and clarifying patient needs, performing clinic‑related patient care, conducting health education, promoting patient advocacy, coordinating health‑care services, and evaluating patient outcomes. The RN also provides support to assigned physicians, communicates with patients/families/caregivers, verifies orders, and ensures follow-up.

Responsibilities

Manage patient flow, triage, and direct patient care.

Act as liaison between front desk staff, nursing staff, physicians, and clinic manager.

Develop expected patient outcomes, define appropriate resource utilization, and set patient goals in collaboration with providers.

Provide nursing support/coordination to optimize patient outcomes and improve quality of life through clinical and educational programs.

Support patient self‑management and coordinate continuity of care post‑discharge and ER visits.

Manage high‑risk patient care, including patients with multiple comorbidities or high risk for readmission.

Use assessment skills to identify physical, psychosocial, financial, and environmental needs and develop relationships with patients.

Educate patients and ancillary personnel about health needs, safety issues, and benefits including patient education.

Document assessments, clinical findings, treatment plans, and progress in EMR, maintaining and developing patient records in compliance with practice standards.

Observe, record and report patient reactions to drugs and treatments; educate about medications, nutrition, and wellness.

Maintain patient confidentiality.

Respond to incoming patient treatment‑related phone calls and instruct patients/families accordingly.

Perform professional, clinical, and technical competencies of the assigned unit or department.

Other duties may be assigned as required.

Required Education & Experience

High School Graduate

Bachelor’s Degree in Nursing

2+ years of RN experience

Basic Life Support Heartsaver (BLS HS) (Non‑Clinical) – AHA or obtained within 14 days

Current active multi‑state RN license in good standing (or single state RN license with proof of application for multi‑state)

Preferred Education & Experience

Previous medical office experience

Required Licensure & Certification

Licensed Registered Nurse (LRN) – Multi‑State – State Board of Nursing

Must be in good standing as of application

Knowledge Requirements

Basic typing and word processing ability

Excellent analytical, critical thinking, and problem‑solving skills
