The Front Door Diversion Quality Assurance & Care Coordinator is a member of Thresholds Intake Department. The focus of the team is on member satisfaction, service efficacy and availability and member outcomes. The team ensures that funder expectations are being met in service delivery and are available to offer support, guidance and deliver hands‐on training for improvement.
The Front Door Diversion Quality Assurance and Care Coordinator serves as the liaison between the agency, IDHS and subsidy administrators. The Quality Assurance and Care Coordinator monitors service delivery and outcomes by maintaining frequent contact with the community service providers, the Thresholds treatment teams, subsidy administrators and direct contact with the members. This role is responsible for the reporting of member transition status and outcomes to all parties involved.
