Job Summary
The Psychiatric Services Case Manager is dedicated to advocating for clients' health, ensuring they receive personalized and appropriate care. This role involves collaborating with healthcare professionals to support the development and implementation of effective treatment plans and managing duties assigned by the Psychiatric Services Manager. The Case Manager plays a pivotal role in coordinating care, providing support, and ensuring continuity of care for clients transitioning both into and out of outpatient settings.
Provides high-quality service and maintains 50% of direct service hours per month
Complete Service Ticket/Progress Note within three working days of day of service
Timely completion of all required documents (medication consent forms, release of information forms, lab request forms) within three business days of completed provider note and recommendations
Complete referral, transfer, and discharge paperwork in a timely manner: referral and transfer, immediately upon need; discharge summary and instructions within three working days of discharge
Under direction of psychiatric providers, case manager will provide clients medication education print outs that are focused on understanding illness, symptom management and medication compliance within three business days of completed provider note and recommendations
Timely completion of client onboarding process after initial evaluation (telehealth education, client portal support, schedule follow up appointments as needed, review necessary documentation and department expectations)
Take client vitals during in-person appointments
Requirements
Bachelor's degree in a related field
Working experience in a mental health outpatient or community healthcare setting
Competency in Microsoft Office365 applications including Word, Excel, Outlook and Zoom
Strong written and verbal communication skills
Highly organized and efficient in managing several cases at once
Experience with SmartCare or a similar Electronic Health Record system (EHR)
Bilingual in English and Spanish, with complete fluency in speaking, reading, and writing both languages (highly preferred)
