Psychiatric Emergency Services Case Manager
Riverbend Community Mental Health, Inc. is seeking a dedicated and organized Case Manager to join our Psychiatric Emergency Services team at Concord Hospital .
Location: Concord Hospital Emergency Department
Schedule: Full-Time | Monday–Friday | 8:00 AM – 5:00 PM (1‑hour lunch break)
Pay: $20.43 – $26.54 per hour
Status: Non‑Exempt
What You’ll Do
Lead daily psychiatric rounds to support patient disposition planning
Obtain necessary releases of information (ROI) and manage documentation efficiently
Act as a liaison by coordinating communication with internal and external providers
Assist with hospital admissions and facilitate smooth inpatient placement, including organizing records, labs, and necessary documentation
Track and manage the inpatient queue, including insurance authorization and EMTALA compliance
Collaborate with medical teams to ensure all needed labs and assessments are completed
Organize and coordinate meetings with family members, guardians, and providers to support care planning
Promote effective use of clinical resources and strengthen connections with community supports
Work closely with clinical and administrative teams to ensure efficient discharge and placement processes

Work Environment
This position is based at Concord Hospital , with a shared office space within the Emergency Department. You’ll work across hospital departments, coordinating closely with medical and psychiatric teams.

Qualifications & Experience
Bachelor’s degree in Psychology, Social Work, Education, Rehabilitation, Human Services, or a related field
A valid driver’s license, reliable personal vehicle, and current auto insurance (or reasonable alternative transportation)
Strong communication, problem‑solving, organizational, and time‑management skills
A solid understanding of inpatient and outpatient systems of care across the lifespan
Ability to obtain and maintain credentialing at Concord Hospital
