About the role
The PRN Registered Nurse Case Manager plans, organizes, and directs hospice care and is an experienced nurse, with emphasis on community health education/experience. The professional nurse builds from community resources to plan and direct services to meet the needs of individuals and families within their homes and communities.

Job Responsibilities

Complete an initial, comprehensive assessment of the patient and family to determine hospice needs, including a complete physical assessment and medical history.

Provide professional nursing care by utilizing all elements of the nursing process.

Assess and evaluate patient status by writing and initiating a plan of care and regularly re-evaluating patient and family/caregiver needs.

Participate in revising the plan of care as necessary; initiate and revise the plan of care as patient status changes.

Use health assessment data to determine nursing diagnoses and develop care plans that include patient and family in the planning process.

Initiate preventive and rehabilitative nursing procedures; administer medications and treatments as prescribed in the physician’s plan of care.

Counsel the patient and family in meeting nursing and related needs; provide health care instructions as appropriate.

Assist the patient with activities of daily living and support self-sufficiency and comfort care when appropriate.

Act as Case Manager when assigned by Clinical Supervisor and coordinate patient care for an assigned caseload.

Communication

Complete, maintain, and submit accurate clinical notes regarding the patient’s condition and care; record pain and symptom management changes/outcomes as appropriate.

Communicate with the physician regarding patient needs and changes in condition; obtain physician orders as required.

Coordinate with community health professionals to align the care plan; teach patient and family self-care techniques; work with the interdisciplinary team to provide safe, coordinated care.

Provide a safe environment for the patient and support continuity of care with family/caregivers and the hospice team.

Attend interdisciplinary group meetings and collaborate with the team to meet the emotional needs of the patient and family.

Additional Duties

Participate in on-call duties as defined by policy.

Ensure arrangements for equipment and services are available; supervise ancillary personnel as required.

Commit to personal growth and maintain up-to-date professional knowledge through continuing education.

Fulfill assigned case work and participate in quality assessment and performance improvement activities.

Job Qualifications

Graduate of an accredited school of nursing. One to two years of recent acute care experience in an institutional setting and one to two years of home care experience preferred.

Current licensure in the state and CPR certification.

Bachelor’s degree, with one year of hospice care experience preferred.

Management experience is not required; may be responsible for supervising hospice aides.

Excellent observation, verbal and written communication skills; strong problem-solving and math skills; nursing skills as per competency checklist.

Physical ability to lift, position, and transfer patients; ability to lift supplies and equipment; ability to stand/walk for long periods as required.

Must be a licensed driver with a valid, insured vehicle in good working order.
