Registered Nurse Case Manager

We are currently seeking a Registered Nurse (RN) Case Manager who is passionate about advancing better living and care for seniors.
What We're Looking For:
- Current and active RN license
- 1-2 years of HH experience-required
- OASIS knowledge-required
- Strong assessment and documentation skills
- Dedication to providing exceptional patient care
Essential Functions:
- Develop, implement, and update individualized plans of care, incorporating physician orders, medications, and input from clients, caregivers, and representatives.
- Complete comprehensive client assessments, including physical exams and health histories.
- Provide direct patient care, including preventive and rehabilitative procedures, medication administration, and skilled nursing interventions.
- Communicate with physicians/physician extenders regarding client conditions, updates, and care needs.
- Educate and mentor clients, caregivers, and representatives on disease management, end-of-life care, and home care needs.
- Collaborate with the interdisciplinary team to ensure coordinated care, discharge planning, and ongoing eligibility evaluations.
- Document all care accurately and in compliance with regulatory and organizational standards.
- Supervise and evaluate LPNs, LVNs, and Home Health/Hospice aides to ensure competency and adherence to care plans.
