About the role The PRN Registered Nurse Case Manager plans, organizes, and directs hospice care. The professional nurse leverages community resources to plan and direct services in patients' homes and communities, with emphasis on community health education and experience.
Job Responsibilities Complete an initial, comprehensive assessment of the patient and family to determine hospice needs, including a full physical assessment and history of current and previous illness(es).
Provide professional nursing care by utilizing all elements of the nursing process.
Assess and evaluate patient status, write and initiate a plan of care, regularly re-evaluate needs, and revise the plan of care as necessary.
Initiate and revise the plan of care as patient status changes; determine nursing diagnosis from health assessment data.
Develop and implement care plans with goals based on nursing diagnosis, involving the patient and family in planning.
Initiate preventive and rehabilitative nursing procedures; administer medications and treatments as prescribed by the physicians plan of care.
Counsel patient and family in meeting nursing and related needs; provide health care instructions as appropriate; assist with activities of daily living and support self-sufficiency and comfort care.
Act as Case Manager when assigned, coordinating patient care for the caseload.
Communication Complete and submit accurate clinical notes; document pain/symptom management changes and outcomes.
Communicate with the physician regarding patient needs and changes in condition; coordinate with community health professionals to align the care plan.
Educate patient and family in self-care techniques; provide medication, diet, and other instructions as ordered; work with the interdisciplinary team to ensure continuity of care.
Maintain a safe environment for the patient and support continuity of care with family/caregivers and the hospice team.
Attend interdisciplinary group meetings and participate in coordinating emotional support for patient and family.
Additional Duties Participate in on-call duties as defined by policy.
Ensure arrangements for equipment and services are available; supervise ancillary personnel when required.
Commit to ongoing professional development through continuing education and in-service classes.
Participate in quality assessment and performance improvement activities.
Job Qualifications Graduate of an accredited school of nursing; current licensure and CPR certification in the state of practice.
Bachelors degree preferred, with hospice care experience; one to two years of recent acute care and home care experience preferred.
Experience in hospice care and management; supervision of hospice aides as appropriate.
Excellent observation, communication, problem-solving, and basic math skills; nursing skills per competency checklist.
Physical ability to perform duties including walking, lifting, bending, and transferring patients; ability to lift up to 50 pounds as required.
Must be a licensed driver with a reliable automobile in good working order.
