Post Acute Network, Care Manager
Full-time position at Duly Health and Care. 40 hours per week, Monday through Friday, 8:00 a.m. to 5:00 p.m.; flexibility for later hours as needed.
Compensation
Base pay range $67,000 – $101,000. Total rewards will also include shift differentials, bonuses, and other incentives.
Overview

The Post Acute Network Care Manager works with skilled nursing facilities to promote quality outcomes for patients with complex medical needs, ensuring appropriate level‑of‑care, and preventing readmissions.
Responsibilities
Manage a weekly caseload of patients with complex medical needs across multiple skilled facility networks
Review, identify, and work to remove patient barriers to discharge through collaborative discussions and resources
Identify patients before discharge for additional Duly services and coordinate, preparing them for a successful discharge
Communicate and elevate patient or facility issues as they arise
Facilitate patient handoff report to Duly CM for continued support when discharged from the skilled facility
Coordinate health‑care services with physicians and other providers as applicable
Manage and provide a comprehensive summary and log of all active patients
Perform additional duties/tasks as assigned
Qualifications
Licensure / Certification
Active Registered Nurse License in the State of Illinois in good standing
Certification in Case Management preferred
Current CPR certification (BLS) required
Education
Associate’s degree in nursing required; Bachelor’s degree in nursing preferred
Experience
2–4 years of experience as a Registered Nurse in a primary care or hospital setting
Previous experience in case management preferred
