Responsibilities
As a Pharmacy Care Coordinator, you will:
Conduct high-volume outbound calls to members using an auto dialer to address medication non-compliance, schedule pharmacy appointments, and gather follow-up information from providers
Handle inbound calls from members, assist with inquiries, and route calls appropriately
Validate member demographic information and follow system scripting protocols
Accurately document member and provider interactions in the system
Support PCPs and clinics with quality data collection and performance monitoring
Collaborate with local leadership to implement quality strategies and engage clinic staff in closing care gaps
Maintain up-to-date knowledge of HEDIS/STARs standards and guidelines
Provide reports and recommendations based on data analysis and chart reviews
Identify patients with missing measures and work to close gaps through education and appointment coordination
Perform other related duties as assigned

Required Qualifications
Prior experience with HEDIS/STARs or similar regulatory reporting
Minimum 3 years of healthcare experience, preferably in a managed care setting
Minimum 3 years of experience in data analysis and quality chart reviews
Proficiency in Microsoft Office applications, including Word, Excel, and database tools
